MO WORK PERMIT INSTRUCTIONS

  1.  Download the following form - Missouri Department of Labor work permit - Entertainment Industry PDF, located at the bottom of the page.

  2. Fill out the form with your information (see example below).

  3. VERY IMPORTANT: Be sure to CHECK THE BOX “Yes”, to request that the hour limitations be waived.

  4. You will need to provide a copy of the birth certificate or other form of proof of age. This can be a picture or a scan of the document.

  5. Email the Department of Labor the form and also attach the proof of age document. Request that the Missouri Department of Labor email you a copy of the work permit.

** Note that the Missouri Department of Labor work permit lasts for one year. You have to apply for a renewal of the work permit once it expires.
